Swelling and joint pain in the hands can be due to arthritis, carpal tunnel, chronic inflammation etc. Gout is known to commonly affect the big toe and knee joints first. It would be best to get a complete check-up done by a Orthopedic doctor for diagnosis. Gout can be easily detected via tests. Regarding pain behind the knee, such pain is frequently due to tendonitis or popliteus muscle injury/strain. Chronic pain should not be ignored and needs a consult right away.
