I would think pollution levels are higher in urban cities compared to rural but the kitchen can be quite polluting in most villages. The biggest factor would probably be smoking and I don't know if smoking levels vary between rural and metro areas

I agree that Pollution is greater in urban areas and so is smoking at least in India. This is because the rural population is heavy on chewing(smokeless) tobacco.
