Hi. Thanks for the response.
What are shingles? We have shown him to many neurologists but none of them seem to have correct answer to his problem. Some says it is trigeminal neuralgia but whichever medicines we gave him benefits for only 1-2 week then he is back to his old condition. He had a history of such pain in his young age when he got his tooth plucked but the pain got corrected by itself but the pain which he is suffering from now since last 5 years after getting his tooth plucked in 2010 in getting unbearable with days coming.

Jaw and neck pain can be due to neurological (such as TMJ, neuralgia, nerve impingement etc), dental issues, angina in acute cases, and even osteoarthritis. Since neurology assessment has not found a cause, it may benefit from looking into other differentials. It's also possible that his last dental treatment may have caused some permanent nerve compression or damage.

Shingles is a acute inflammation of the nerve and the surrounding skin area present with rash, blisters, itching etc. It is an infection caused by the varicella zoster virus. It is usually seen in the elderly or people with low immunity. However this is an acute condition that subsides after 2-4 weeks.

Thanks Doctor Shital for your kind response. We also did tried exploring other angles such as getting his teeths plucked which have become bad but since he is now 75, he prefers to abstain from any surgery as he is afraid that it might aggreviate the pain if it does not treat the cause. We have tried doing the pscyiotherapy as well but that also did not benefitted him. He is used to the pain now which may happen at as early as 2 AM in the morning and will continue for 15-20 minutes and subsides by itself. He applies ice pack on the part where the pain happens but not much benefit it provides either. The pain that starts in the morning keeps occuring till around 12 PM or so and then the intensity reduces by itself. He is OK in intervals and we have noticed the trend that he gets pain when he is trying to have his meals. It is a very typical kind of pain which none of us / any in our neighbourhood have seen in recent past.

According to Dr. Bhuvaneswari, there are 2 possibilities:
1. Nerve traction /,injury during dental extraction .
2 .salivary gland calculus /partial duct stricture.
MRI salivary gland or a siaogram ( injecting dye into salivary duct and staying) can help.
Apart from neurotropics , some bio feedback / anti depressants might help

Thanks for your reply. I am not too sure what shall be effect of the medications suggested above. I am from Mumbai, India. Let me know if we can show him to some of the doctors whom we have not gone through earlier. We have already been to big hospitals such as Kokilaben Ambani Hospital plus some other neurologists. We also gave him some anti depressants but they were also ineffective plus when he was on those doses he was most of the time in sleepy kind of situation which we did not preferred. Also one of the doctors also suggested not to give these for a prolonged period. Let me know. Thanks.
