Diabetes Maintenance Schedule

Diabetes requires regular testing, monitoring, consultation at different time intervals. A simple schedule of follow ups and vaccines needed, helps to keep on top of things.

 

Note: Use this as a guide but follow your doctor’s prescribed schedule

Every Day

Take your medicines on time.
Check your blood glucose levels with a Glucometer. Maintain a log of time and readings. Share with your doctor at next consult.
Check your blood pressure with a home BP monitor as prescribed by your doctor or if you sense fluctuations. 
Follow foot care routine daily and check foot daily. 
Exercise for a minimum of 30 minutes a day.
Stick to a sensible diabetes friendly diet. Limit sugar, salt and fat intake.
Practice stress management techniques like yoga, meditation, hobbies, etc.

Every 3 months

HbA1c if your treatment has recently changed or your A1c has not been in control, ideally below 7%.
Doctor’s visit especially if your blood glucose and blood pressure levels are not under control. 
Share your log of blood glucose and blood pressure readings with the doctor.

Every 6 months

HbA1c to keep an eye on your levels, which should be ideally below 7%.
Clinic visit as prescribed by your doctor or Diabetes nurse/educator.
Dental visit for simple cleaning and check-up of gums and oral health.
Share your log of blood glucose and blood pressure readings with the doctor.

Every 1 Year

Laboratory blood testing of lipids, kidney profile, blood sugars and complete blood count.
Foot examination by your Diabetes care team. 

Take the annual influenza vaccine 

Ask your doctor when you need to start on the pneumococcal vaccine
